“What role should the Community Center play in HVL?” is reflected in this survey. The Commons Committee will compile the answers to get a better understanding of members’ wishes for the future of the Community Center.

The two-page questionnaire can be downloaded by selecting the image in the left column above. Please complete and email it back to the email address above (media@hvla.com.) Hard copies can be submitted to, or obtained from the Administration Offices.

EMAILING INSTRUCTIONS - After completing the form on your computer: if you use an installed email program that resides on your computer, (Outlook, Mac Mail, Entourage, etc.) simply click "Submit Form" and follow the directions. If you use a web-based email program (gmail, hotmail, etc.) SAVE it as you would any other file or document. Then create a NEW email addressed to media@hvla.com, ATTACH your completed survey to this email and SEND it.

If you're have trouble saving the document, try saving it as a copy with another name. Alternately, you can print out the survey, complete it manually and snail mail or hand deliver to the Administration Offices.
